MAINTAIN OR MAKE CHANGES TO AN EXISTING WIDE BAY AUSTRALIA ACCOUNT

What do you want to do?

Add or remove a Signatory/Authority to Access on an existing account

Step 1 - Complete and sign an Account Maintenance (PDF, 74KB) form in the appropriate place. ALL existing (and new if applicable) account Signatories (and in some cases Authorities to Access) are required to sign.

(Note:  The first Signatory (accountholder) cannot be removed without closure of the account – if you wish to remove the first signatory please contact Wide Bay Australia.)

Step 2 - New Signatories/Authorities to Access will need to complete and sign an Individual Client form (PDF, 49KB) form.

Step 3 - New Signatories/Authorities to Access will also need to provide Wide Bay Australia with evidence of their identity (as required by the Anti-Money Laundering and Counter-Terrorism Financing (AML/CTF) Act 2006). (Identification is not required if you are a Signatory/Authority to Access on an existing Wide Bay Australia account and you have previously provided identification).

Your identity can be verified by you providing your relevant identity documents in person at any Wide Bay Australia branch. Examples of acceptable identification documents are outlined on the Identification Requirements under the Anti-Money Laundering and Counter-Terrorism Financing (AML/CTF) Act 2006 (PDF, 35KB). If you are sending your application by mail and you are unable to visit one of our branches, you will be required to lodge at your nearest Australia Post outlet, an Australia Post – Identity Verification Form (PDF, 155KB) along with your original and current identification documents as detailed on this form.

Clubs/Organisations: In addition to providing individual identification, in cases of clubs and organisations changing signatories, we will require a copy of minutes or a letter on letterhead stating the appointment of new office bearers and detail the nominated signatories for accounts and the removal of the previous office bearers and or nominated signatories.

Step 4 - Once complete please return forms to your nearest Wide Bay Australia branch; or post to: Reply Paid 1063, Bundaberg QLD 4670; or alternatively, fax through to (07) 4152 3499.

Freeze (Block) or Unfreeze (Unblock) transactions on an existing account

Complete and sign an Account Maintenance (PDF, 74KB) form in the appropriate place. The required number of account Signatories are to sign to block an account. ALL signatories are required to authorise an account to be unblocked.

Add an account name suffix (eg additional description to be added to your account name) or label your account with a description (eg Car Account, Holiday Account etc)

Complete and sign an Account Maintenance (PDF, 74KB) form in the appropriate place. The required number of account Signatories are to sign.

To change your account name please contact Wide Bay Australia.

Once complete please return forms to your nearest Wide Bay Australia branch; or post to: Reply Paid 1063, Bundaberg QLD 4670; or alternatively, fax through to (07) 4152 3499.

Change the number of existing Signatories required to sign on an account

Complete and sign an Account Maintenance (PDF, 74KB) form in the appropriate place. ALL EXISTING account Signatories are required to sign

If you are a club/organisation please refer to the Change of Signatories to Clubs and Association Accounts (PDF, 37KB) for instructions on changing the Signatories on your account.

Change address or contact details for a personal account.

Each Signatory/Authority to Access on an account can nominate their own address and contact details. However the account address/contact details will always be the same as the first Signatory (accountholder) unless nominated otherwise. To change the account address and contact details the first Signatory should complete and sign a Client Address/Contact Details Maintenance (PDF, 38KB) form.

Note: If the address/contact details of other Signatories/Authorities to Access (apart from the first Signatory) are also to change then each Signatory/Authority to Access must complete and sign an individual Client Address/Contact Details Maintenance (PDF, 38KB) form.

Change address or contact details for an account held by a business/organisation

Complete and sign a Client Address/Contact Details Maintenance (PDF, 38KB) form. The required number of account Signatories are to sign.

Change address or contact details for an individual account Signatory/Authority to Access

Complete and sign a Client Address/Contact Details Maintenance (PDF, 38KB) form.

Once complete please return forms to your nearest Wide Bay Australia branch; or post to: Reply Paid 1063, Bundaberg QLD 4670; or alternatively, fax through to (07) 4152 3499.

Please allow 3 working days from receipt by Wide Bay Australia for processing.

Note: The account address/contact details will always be the same as the first Signatory (accountholder). If you are the first Signatory (accountholder) - changing your address/contact details will change the account address/contact details.

Change the name of a Signatory/Authority to Access on an existing Wide Bay Australia account

Complete and sign a Client Maintenance (PDF, xxKB) form.   

Evidence confirming the change of name will need to be supplied with the Client Maintenance form.  For example, a Marriage Certificate in the case of a woman adopting her husband’s surname. This evidence could be an original if presenting in person at a Wide Bay Australia branch or a copy certified by a Commissioner of Declarations/Justice of the Peace if supplying by mail.

Change the existing Client signature on your account

Complete and sign a Client Maintenance (PDF, xxKB) form.

When updating your client signature, you must sign the form with your existing signature as well as your new signature.

If you are sending your Client Maintenance form, you must also attach a certified copy of photo identification (eg Drivers Licence, Student Card, etc). This must be certified by an acceptable referee (PDF, 28KB).
